Position Description

Job Summary

The Nurse Practitioner, as a primary care provider, is responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders. The Position will require working on a mobile clinic with patients from local schools, faculty and people from the community. The mobile clinic currently is at 4 locations. The mobile clinic is also part of the Emergency Plan for the City of Pasadena and could see patients during a hurricane or other type of disaster.

The nurse practitioner will supervise staff on the mobile clinic and will serve as a clinical coordinator. This is a solo job with an MA so it is important that the FNP is confident and experienced. There will be other assisgned patients during the week at the clinic and this is also part of responsibilities. 

Duties and Responsibilities

The Family Nurse Practitioner performs the following duties as assigned.

  1. Provides clinical services.
  2. Maintains clinical expertise and competency (to include age specific competency) as well as demonstrates thorough knowledge of procedures to deliver quality primary health care.
  3. Documents delivery of health care and nursing process in a timely, accurate, and consistent manner.
  4. Educates the patient and family/significant others about diagnosis and the plan of care. Acts as a resource for facilitating communication when necessary.
  5. Coordinates the delivery of patient care provided by the health care team.
  6. Interacts with health center personnel and provides optimal patient care and works to establish obtain optimum patient flow and increased patient satisfaction.
  7. Identifies and responds promptly to changes in patient’s condition and/or clinic environment.
  8. Coordinates and delegates the timely processing of patient referrals, prescribed medications, and other identified needs of the patient.
  9. Acts as a resource within the clinic to other staff members in regards to patient care.
  10. Participates as a team member and promotes teamwork by assisting others with patient care and department activities.
  11. Administers all medications accurately using the five (5) rights of medication administration, documenting medications according to health center policy and procedures.
  12. Upon completion of laboratory skill assessment, performs all laboratory functions according to licensure.
  13. Provides clinical oversight under the supervision of the Medical Director.Assumes responsibility/accountability for delegating, directing, and supervising patient care activities.
  14. Identifies Performance Improvement opportunities.
  15. Serves as a resource and positive, proactive leader for the department.
  16. Accepts responsibility for remaining current with nursing and health care trends and information that impact nursing practice.
  17. Participates in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care.
  18. Participates in the evaluation of products, procedures, and patient care standards.
  19. Participates in the evaluation of health care at the Center and in monitoring health outcomes of patients.

Responsibilities include, but are not limited to:

  1. Triage, examine, diagnose and treat patients
  2. Counsel and educate patients about acute and chronic conditions or concerns
  3. Collects record and maintain accurate patient information
  4. Document items such as: appropriate chief complaint, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan
  5. Facilitate provider referrals when appropriate
  6. Formulate diagnostic and treatment plans
  7. Complete all documentation and paperwork in a timely manner within the electronic medical record
  8. Work with the media/communication team to promote the clinic
  9. Other duties as assigned
  10. Fulfills administrative duties.Assists in maintaining an efficient, safe, secure, orderly, and aseptic environment of care for all patients and employees.
  11. Is familiar with and abides by OSHA Standards, Hazardous Materials, Universal Precautions, use of personal protective equipment, Evacuation, Fire, Disaster, and Safety Plans.
  12. Performs all other duties as assigned. 

Knowledge, Skills and Abilities

Required

  • Knowledge of basic principles of health promotion, prevention and motivation
  • Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center’s current operational protocols
  • Knowledge of medical terminology and appropriate abbreviations
  • Demonstrates knowledge of the Texas Nursing Practice Act
  • Ability to read and comprehend medical instructions, short correspondence, and memos
  • Ability to document, with clarity, all information relevant to a patient’s needs
  • Ability to effectively present information in small group situations to patients, Center staff, and the general public
  • Ability to prepare statistical and administrative reports
  • Ability to work cooperatively with community agency personnel to coordinate services for patients
  • Ability to use a glucometer and interpret the results
  • Ability to use office equipment including but not limited to computers, fax machines, telephones and copiers
  • Ability to prioritize and allocate resources
  • Ability to document HRSA measurements including BMI, Vaccines, etc...
  • Ability to properly use a monitor defibulator and to recognize critical heart rythms.

Preferred

  • Spanish speaking and writing skills

Credentials and Experience

Required

  • Registered Nurse degree from an accredited school of nursing
  • Advanced Practice Nurse degree from an accredited school of nursing
  • Maintains licensure in the State of Texas
  • Current unrestricted license from the Texas Board of Nurse Examiners to practice as a family nurse practitioner (FNP) in the State of Texas
  • Current Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals Certification
  • Prefer Current ACLS and PALS certification

Preferred

  • One (1) year of practice experience as an RN giving direct patient care.

Special Requirements

The employee must be able to lift and/or move more than 50 pounds. Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye/hand coordination.  Requires sitting, standing and walking for extensive periods of time.  Requires working under stressful conditions or working irregular hours.  Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment. The employee frequently is required to reach with hands and arms.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.  Ability to work with a moderate noise level in the work environment is required.
